OVERVIEW
There is no place like home when you are not feeling well. That is why you will find that the Tioga County Certified Home Health Agency and Long Term Home Health Care Program are there for you when you need a helping hand. The goal of home care is to allow you or your loved one to stay at home as an alternative to remaining in the hospital, or another type of institution, and to become as independent as possible in managing your own health needs. You will find that all of our staff will provide friendly and compassionate care to you and your loved ones.
The Tioga County Health Department is certified by the New York State Department of Health to provide a variety of services for those with temporary illnesses or chronic health conditions. Care may be as simple as taking your blood pressure, or as technical as intravenous administration. Whatever the need, our home care staff is prepared to bring quality care into the home on an intermittent basis. Agency personnel work directly under the orders of your physician and are skilled in assisting you and your family in a variety of ways.
ELIGIBILITY
Tioga County Medicare Certified Home Health Agency
-
Illness or disability that can be safely cared for in your home.
-
A physician order for home care and a physician who will follow you during your illness.
-
A condition that requires skilled professional services (e.g., skilled nursing, physical therapy, or speech therapy).

A condition that requires short-term, intermittent care.
-
Must be homebound; this means it is difficult and taxing for you to leave your home.

Must be living in Tioga County at the time of service.
Tioga County Long Term Home Health Care Program
-
Illness or disability that can be safely cared for in your home.
-
A physician order for home care and a physician who will follow with you during your illness.
-
Have a condition that primarily requires the services of non-professional caregivers (e.g., Home Health Aide, Personal Care Aide, Housekeeping).

Must qualify for nursing home or adult residential placement and be able to be maintained at home.
-
Must be living in Tioga County at the time of service.
SERVICES PROVIDED
Skilled Nursing
Upon admission, a Registered Nurse will be assigned to manage your care. Under the direction of your physician, the nurse and you will form your plan of care, based on your individual health needs. Some of the services we provide are:

Patients qualifying for the Long Term Home Health Care Program may also qualify for the following services:

Personal Care Aide – to assist with bathing, dressing, eating.

Housekeeping – assistance with household chores, laundry, and errands

Meals on Wheels – up to two meals/ day; 1 hot, 1 cold
